Notes from Mk Sastry Lecture .

What needs to be known in handling stroke cases, the checks needed are: 
  1. We must know the name of the muscles to be treated, know the location, function and insertion and origins, 
  2. Check MMT, can learn from you tube m. Vastus Medialis m. Lateral Vastus m. Inter medial vastus m. Rectus Femoris All of this is called Quadriceps muscle, see muscle anatomy map! 
  3. If there is spastic in the gluteus / difficulty moving the butt muscles, the way of thinking kits has to go back again. Femoral nerve travel, these nerves supply the lower limbs. find out the quadriceps pathway. Look for the flow of quadriceps muscles and the femoral nerve path (n. Femoralis), After that, it can treat the patient, even injecting fluids into the area. 
  4. Eye Check: Dilated Pupils, S1 and S2.
  5. Heart check: is there a heart murmur 
  6. Cervical APLateral and Dorso  Lumbar AP lateral  X-ray 
  7. Chest X-ray 
  8. ECG
  9. MRI Brain, for Haemoragic Stroke case, 
  10. MRA brain, if there is a suspicion of brain aneurysm, seeing a blockage in the blood vessels of the brain, atherosclerosis, etc. * If the patient's condition is stable, no need for MRI but CT scan
  11. Lab Check: 
    1. Complete Blood (CBC), LED (ESR), 
    2. Blood lipids. (total cholesterol, HDL, LDL,  IDL, triglycerides) 
    3. GDS (When Blood Sugar) 
    4. Urinals (Urine Routine) 
    5. No. 6-11, this is called a supporting examination which helps us to establish the patient's diagnosis to take further action
  12. After all of the above, don't forget to ask about the patient's co-morbid history. >>>> For example: hypertension, asthma, diabetes, etc. 
  13. After all the above is done, then we can examine and treat the patient. 
Case 1. Joy’s patient Male, 85 years old with Hemorrhagic Stroke, Co-morbid: 
    1. Chronic hypertension,
    2. Chronic kidney failure, 
    3. COPD, any other  Chronic  Disease Patients cannot be treated because the supporting data is incomplete, no x-rays, / CT scan or MRI. This is one example where we can not do treatment, because it violates medico legal.
Case 2. Female, 60 years old, with Cervico spondilytis multiple PIVD . With frozen shoulder  Therapy has been carried out as we saw this afternoon, recovered 90%.
